Our Social Worker is based at the Hospice and is on hand to help patients and families get the best quality of life, both during and after illness. The main focus is to help each patient decide what’s best for them and support them in every way they can. From a practical point of view, the Social Worker will assess the patient’s social and care needs before and after discharge, give advice on welfare benefits, additional funding and wills, as well as liaise with other agencies and services if required. We’ll also help with the emotional needs of the patient and family. The Social Worker will explore family dynamics and if necessary arrange a meeting with the family team to help with any difficulties or concerns, people may be experiencing.
Our counsellors are trained and qualified to help with all kinds of difficult experiences and concerns and can help you develop a coping strategy that will work best for you.
