We collect and hold information about you to enable us to give you the correct care and treatment. The information will be kept as a paper record, electronically (for example in electronic patient records or digital images) or both. Records are stored securely within the Hospice. It forms part of your health record and will be kept in case we need to see you again.
Members of the clinical teams looking after you may share your personal health information with each other to provide the treatment as appropriate to you.
These teams may include:
- Healthcare professionals (such as doctors, nurses, pharmacists, physiotherapists and occupation therapists for example)
- Administrative support staff
- Healthcare students in training
- Staff conduct local clinical audits to evaluate the care provided to you.
All staff are bound by law and a strict code of confidentiality. In accordance with NHS guidance, the hospice has appointed a Caldecott Guardian; a senior member of staff who is responsible for protecting the confidentially of patient and service user information and enabling appropriate information sharing.
